- 2 years certified accounts
- Notice of assessment from revenue (form 11/chapter 4)
- Personal statement for the last 3 months
- Farm account statements for the last 6 months
- Statement of direct payments
- Evidence of non-farm income if applicable
- Milk statement for the previous year for dairy farmers
- Confirmation of Tams Grant if applicable
